Well Visits/Health Screening Questionnaires

Well child visits and immunizations are a very important part of maintaining your child’s health. At these visits we look not only for signs of illness or disease, but also we want to make sure that your child’s development is proceeding normally. Please feel free to ask questions at these visits.

Well Visits and Vaccinations

Scheduled well visits and corresponding parent questionnaires at our practice include, please remember to bring the questionnaire to your visit:

More frequent examinations might be required for some children – those in competitive sports or with chronic disease, for example.

Immunizations are generally coordinated with well child visits and are a very important part of preventative care. These vaccines help prevent a number of potentially serious diseases. The American Academy of Pediatrics continually revises their vaccine recommendations. We administer vaccinations based on their recommendations. The most current recommendations can be found in our Vaccines section.

Well child visits are arranged by appointment only. Our office hours are 7:30 – 5:00 Mon.- Fri. When you cannot keep an appointment, please call our office as soon as possible so that we can schedule another child in that visit. We allow more time for our well child visits so that there will be ample opportunity for you to discuss any questions you have about your child’s health and development. However, if you have special concerns you wish to discuss at check-up time please alert our staff when you schedule your appointment so we can schedule adequate time for your visit.

Please try to make every effort to be on time for your appointment. It is our policy to keep your waiting time in our office to a minimum. Accordingly, if you are more than 20 minutes late for a well child checkup we may have to reschedule the appointment so that we can remain on time for our other scheduled appointments. Regretfully, we will not be able to continue to extend care to those families who are habitually late or who fail to show for appointments.

If an emergency arises in our office or at the hospital that will make us unable to stay on schedule we will inform you of these developments and you can reschedule your appointment if you desire. These situations should be quite rare, however.
