New Patients

Whether you are expecting your first child, transferring from another city or just looking for a change, we welcome you to Pediatric Health Care Associates. You will have a primary pediatrician who you will generally see for your child’s Well Child Visits. For the Sick Visits, you may meet different providers due to availability on certain days. If you are transferring from another practice, call our office for an appointment if you need one and let us know you are a new patient. We will also have you talk to our business office to gather your insurance information.

On your first visit, New Patients Should:

Bring with you your entire child’s medical history, shot records, and other information you feel would help us to better care for them.

New Patients should try to arrive at our office a few minutes before your scheduled appointment time in order to complete the New Patient Information packet.

If you are transferring from another physician, we will ask you to complete a medical records request so we can have a complete account of your child’s care.

This will save you valuable time.

Insurance information is vitally important, so be sure to bring your insurance card with you, as well as a picture identification. Caring for your child may require us to contact you from time to time, so accurate and current addresses and phone numbers are important to have.

On New Patients first visit we must have a parent or legal guardian present,

so please arrange this in advance. For future visits, your child may be accompanied by a health care surrogate, but we will need to have you complete a release form IN ADVANCE in order to see your child without you present.
